Please find below the flu letter from Bromley Immunisation Team, which also includes the link to the e-consent form.

When filling in the form, please ensure the correct full school name is selected (there are multiple schools with similar names). Only one parent needs to complete the consent form per child to avoid duplication.

The closing date for the e-consent form is Monday 15th September. If you miss the portal closing date, you will need to contact the Immunisation Team directly.
